What is the NHTD Waiver Program?

The Nursing Home Transition Diversion (NHTD) program is a Medicaid funded program that helps elderly and handicapped who DO NOT want to spend their lives in a nursing home or in an institution.

As is its namesake, the NHTD program is especially good for those transitioning from a nursing home to their own residence, as well as those who are in danger of being placed in a nursing home or institution but want to prevent it.

Under this Home and Community-Based Service (HCSB) qualified participants will receive the opportunity and support needed to live in the larger community.

To be eligible for the NHTD Waiver Program, you must satisfy the following requirements:

Bethesda Elite Care

Age:

  • Individuals aged 18-64

    Must have a documented physical disability.

  • Seniors aged 65 and older

    May apply without needing documented disability but must require at least one waiver service.

  • Medicaid Eligibility

    The applicant must be a Medicaid recipient.

  • Nursing Home Level of Care

    Be evaluated as requiring the level of care offered in a nursing home.

  • Desire to Live in the Community

    You prefer living in the community to a nursing facility or institution.

  • Appropriate Living Arrangement

    You must identify a safe and suitable living environment.

  • Service Availability

    You must be able to access the funding and services offered under the HCBS/NHTD waiver and the New York State Medicaid State Plan.

  • No Participation in Other Waivers

    You cannot be actively enrolled in another HCBS waiver program.

Service Coordination

The coordinating of various NHTD services to ensure you receive the necessary supports to maintain your independence and quality of life in your home  

Independent Living Skills Training Services   

Individually designed services aimed at improving or maintaining your ability to live as independently as possible in the community.  

Home Community Support Services

An individualized plan for providing oversight and/or supervision as a discrete service to maintain the health and welfare of the participant living in the community. Personal Care services are provided to assist with Activities of Daily Living (ADL) such as dressing, bathing, hygiene/grooming, toileting, ambulation/mobility, transferring and eating, and/or Instrumental Activities of Daily Living (IADL) such as housekeeping, shopping, meal preparation, laundry, transportation and telephone use essential to the maintenance of the your health and welfare in the community.

Structured Day Program Services

Individually designed services, provided in an outpatient congregate setting or in the community, to improve or maintain the waiver of participant's skills and ability to live as independently as possible in the community. These services may not exceed 10 hours per day  

Community Integration Counseling 

An individually designed counseling service provided to the participant who is coping with altered abilities and skills, a revision of long-term expectations, or changes in roles in relation to significant others.  

Positive Behavioral Interventions and Support

Services provided to waiver participants who have significant behavioral difficulties that jeopardize their ability to remain in the community of choice due to inappropriate responses to events in their environment

Respite Services

Individually designed services intended to provide relief to natural (informal), non-paid supports who provide primary care and support to a waiver participant for a short time (usually 30 days or less)

Environmental Modifications

This service includes the performance of necessary assessments and project management to determine the type of modifications needed and the assessment that the adaptation has been completed according to the required specification. (Specifically, home modifications and/or vehicle modifications)  

Assistive Technology

The purpose of this service is to supplement the Medicaid State Plan Service for Durable Medical Equipment and supplies, which provides a broad range of special medical equipment and supplies

Respiratory Therapy

An individually designed service, specifically provided in your home, intended to provide preventative, maintenance, and rehabilitative airway-related techniques and procedures 

Peer Mentoring

An individually designed service intended to improve your self-sufficiency, self-reliance, and ability to access needed services, goods, and opportunities in the community.

Moving Assistance Services

Services intended to transport your possessions and furnishings to your home. 

Community Transitional Services

Individually designed services intended to assist a waiver participant to transition from a nursing home to living in the community

Congregate and Home Delivered Meals

An individually designed service which provides meals for you if you cannot prepare or obtain nutritionally adequate meals for yourself, or when the provision of such meals will decrease the need for more costly supports to provide in-home meal preparation

Home Visits by Medical Personnel 

Individually designed services to provide diagnosis, treatment and wellness monitoring in order to preserve your functional capacity to remain in your own home  

Nutritional Counseling/Educational Services

Individually designed service which provides an assessment of your nutritional needs and food patterns, and the planning for the provision of food and drink appropriate for your condition(s), or the provision of nutrition education, and counseling to meet normal and therapeutic needs  

Wellness Counseling

Individually designed service intended to assist the medically stable waiver participant in maintaining optimal health status. (intended for those who do not otherwise have access to nursing services)
